About Sample Collection Under a Witness
Sample collection and consent form creation are performed under the witness of professional staff.
The collected samples and consent forms will be collected by the professional staff.
For adult wards, the guardian must give consent and sign; for persons under curatorship, the curator must give consent and sign.
Also, if the test subject is a minor, a person with parental authority must give consent and sign.
In cases where child custody is being determined, the current person with parental authority or legal representative performs this.
To use the test results in court, sample collection, consent form creation, photographing, and ID verification by professional staff are essential. All test participants must cooperate with all of the following.
- Presentation of identity verification documents and submission of a copy image
- Photographing for identity verification
- Signing and fingerprinting the consent form and application form
For identity verification documents, please present one of the following: a photo ID issued by a public institution (driver's license, passport, health insurance card, basic resident register card). If the subject is a minor, please present a passport, birth certificate, maternal and child health handbook, or health insurance card.
