① I would like it to be clearly stated on the consent form that "the child's (or the guardian's) signature is required."
② When resending documents, I would like to be notified by email or otherwise about "where it was sent."
It takes a bit of extra effort, but I think it would be even more helpful if you would consider the above. Just for your reference.
