Prior Confirmation and Consent Form for DNA Testing
- It is also possible to choose not to undergo the testing by seeDNA Inc. (hereinafter referred to as 'the Company').

1Purpose of Testing
The DNA relationship testing performed by the Company is intended to confirm biological relationships.
2Sample Collection
The client shall provide correct information according to the instructions provided by the Company and collect samples carefully to avoid any mix-ups.
3Testing Methods
- Samples submitted by the client will be anonymized by the Company and used to confirm relationships at the Company or its partner testing facilities (Note 1).

Partner 2: DowGene DNA Testing Company Co., Ltd. (Seoul, South Korea) - The test analyzes specific regions of the DNA sequence called 'STR' (Short Tandem Repeat), where short sequence units are repeated. DNA obtained from the sample is amplified using the PCR (Polymerase Chain Reaction) method, and relationships are determined by comparing STR types at 15 to 30 locations, including those specified by the US FBI (Federal Bureau of Investigation).
4Consent to Test Results
- If the subject has received a blood transfusion or bone marrow transplant, or if the subjects are closely related, accurate test results may not be obtained, so you must always inform the Company.
- I understand that there is a possibility that correct test results may not be obtained due to reasons beyond the Company's control, such as mutations, deletions, substitutions, or chimerism in the subject's DNA. I agree not to make any claims against the Company even if correct results cannot be obtained.
- The results of this test are strictly biologically and statistically analyzed, and I understand that theoretically, 100% accurate testing is impossible. I also understand that depending on the condition of the sample, correct test results may not be obtained.
- I understand that the test is conducted based on the information provided by the client, and depending on the test results obtained, it may cause disadvantages to the client and the subject.
5Expected Result Report Date
- The expected result report date is calculated with the business day the sample arrives at the Company as Day 0. If the sample arrives in the afternoon, 1 business day will be added. Business days are weekdays only, excluding Saturdays, Sundays, and public holidays.
- The result report date may fluctuate due to reasons beyond the Company's control, such as the condition of the sample, postal accidents, or damage to the sample due to postal delays, but the Company shall bear no responsibility.
6Reporting Test Results
- Test results will be reported by the method specified at the time of application (document or web page).
- The relationship between the subjects will be reported as a probability of paternity/relationship representing the reliability of the test, or a judgment result based on the Company's statistical criteria.
- To prevent the test results from being communicated to anyone other than the client, we cannot respond to inquiries about test results by phone or facsimile.
- Test results can be checked via the web page (member page) for up to 1 year from the result report date.
- It is possible to send a written result report for up to 3 years from the test result report date.
- We cannot inform you of the progress status during the testing.
7Cancellation of Testing
- In the case of cancellation due to the client's convenience, a full refund will be provided before the sample collection kit is shipped.
- For cancellations due to the client's convenience after the kit has been shipped, 70% of the total request amount will be refunded only if the unopened kit is returned to us.
- No refunds will be given if cancelled after the kit has been opened.
- If the test cannot be completed within 1 year from the application date of the test, it will be considered a cancellation due to customer convenience, and no refunds will be given.
8Free Retesting
- If the first test results in an 'indeterminable' judgment, you can choose to either take a second or subsequent test as a free retest or receive a refund of 30% of the total request amount without retesting.
- If you proceed to the second or subsequent test, no refunds will be given.
- In principle, the sample to be submitted for the second or subsequent test is limited to buccal epithelium. If you submit a special sample, a special sample fee will be charged separately, and the testing period will be extended.
9Expenses Other Than the Test Fee at the Time of Application
If additional charges are incurred due to the client's convenience, such as re-shipping the sample collection kit or issuing an additional written result report, in addition to the requested test, we may charge the fee separately from the test fee at the time of request.
10Other Matters Regarding Refunds
- If the result report date is delayed due to reasons beyond the Company's control, such as the condition of the sample, postal accidents, or damage to the sample due to postal delays, option fees will not be refunded.
- When refunding due to cancellation by the client's convenience or refusal of free retesting, bank transfer fees (or credit card payment fees in the case of credit card payment) will be borne by the client.
- In the unlikely event that there is an error in the test results due to a mistake by the Company, the Company will fully refund the test fee received from the client, and the client agrees not to make any claims exceeding the refunded test fee.
11Storage of Samples and Personal Information
- Emails and personal information from clients will be anonymized and used to improve services.
- Samples and data obtained from samples will be anonymized and used for research and improvement of testing accuracy.
- Samples for which analysis and research have been completed will be discarded by designated contractors without notification or permission from the client. If you wish to store or return the sample, you must contact the Company before the testing starts.
12Disclaimer
- The Company assumes no responsibility for judgment errors caused by damage to the sample, writing errors in documents by the client, or errors in sample collection.
- The Company assumes no responsibility for judgment errors caused by reasons beyond the Company's control, such as mutations, deletions, substitutions, or chimerism in the subject's DNA.
- Under no circumstances can we assume responsibility beyond the refund policy above (see items 7-10).
